Class of 2026 5th Grade Activities Package payment can be made here.

Directions for Completing the Form:

Choose Quantity of 5th Grade Packages (usually this is 1)

If you would like to donate toward general 5th grade fundraising you are welcome to!!

Copy of the form is available here if you wish to print, complete, sign and return with your check or cash. When you drop off your donation, please return the bottom portion with your payment by October 17th, 2025. You can include cash or check made out to Cleveland PTA.
